Alfacalcidol was developed in the 1980s as a vitamin D analogue and is widely used to treat bone disorders. It is a prodrug. In the liver it becomes calcitriol, the active form of vitamin D. This helps your gut absorb calcium and supports bone health.
It acts on the vitamin D receptor in many tissues. This helps balance calcium and phosphorus in your blood. It also lowers parathyroid hormone, or PTH, which can pull minerals from bone. Because it is activated by the liver, it behaves differently from calcitriol made in the kidney.
Common uses include secondary hyperparathyroidism in chronic kidney disease. This condition happens when the parathyroid gland makes too much hormone. Alfacalcidol helps bring calcium and phosphate into balance and supports bones. It is often used when other vitamin D medicines do not help enough.
Doctors may also use it for certain bone conditions tied to vitamin D deficiency. In some cases it helps osteoporosis when other treatments are not enough. Your doctor will decide if this medicine is right for you.
You usually take alfacalcidol by mouth, once daily. Follow the exact dose your doctor writes. Take it with a meal or a glass of water to aid absorption. Swallow the capsule or tablet whole.
If you miss a dose, take it as soon as you remember, unless it's almost time for the next one. Do not double up to catch up. If you are unsure what to do, contact your clinician.
This medicine raises calcium levels in your blood. Too much calcium can be serious. Watch for symptoms like nausea, vomiting, weakness, confusion, or thirst. Keep regular blood tests as advised by your doctor.
Tell your doctor about other medicines you take, including calcium supplements and vitamin D. Some drugs can interact with alfacalcidol, such as thiazide diuretics, digitalis, or certain phosphate binders. Your doctor may adjust your dose or timing.
Most people tolerate alfacalcidol well. Common side effects include stomach upset, nausea, and dry mouth. Dizziness, headache, or general weakness can occur.
Less common but important effects include signs of high calcium, such as excessive thirst or urination, confusion, or muscle pain. If you notice these, contact your doctor promptly.
