What Chlorthalidone is and how it works

Chlorthalidone is a thiazide-like diuretic. It inhibits the Na+-Cl- cotransporter in the distal convoluted tubule, promoting natriuresis and mild diuresis. The subsequent plasma-volume reduction is accompanied by a gradual decrease in peripheral vascular resistance. This mechanism supports blood pressure lowering over time.

Compared with some other diuretics, chlorthalidone has a longer half-life and a more durable antihypertensive effect, enabling once-daily dosing. It may cause modest changes in potassium, uric acid, and glucose metabolism, requiring routine laboratory monitoring. This profile contributes to its use in long-term risk reduction strategies.

Common medical uses

Common medical uses include essential hypertension where it lowers blood pressure and reduces cardiovascular events. It is also used to treat edema associated with heart failure, cirrhosis, or nephrotic syndrome, and may aid in volume management in other fluid-retaining states. It can help address mild hyponatremia in selected patients when appropriate.

In addition to edema control, chlorthalidone is frequently employed in combination regimens to treat hypertension. It is compatible with many agents, including ACE inhibitors, ARBs, and calcium-channel blockers, and contributes to long-term cardiovascular risk reduction in high-risk patients. Its fixed-dose usage may improve adherence in some regimens.

How it is typically taken

Chlorthalidone is administered by mouth as tablets. It is taken once daily, typically in the morning to minimize nocturia. Food does not markedly alter absorption, but a consistent taking pattern improves tolerability and adherence.

Dose is individualized. A starting range of 12.5–25 mg once daily is common, with careful titration up to 50 mg as needed. In renal impairment or in elderly patients, slower titration and periodic monitoring of electrolytes are advised. Patients should discuss any planned changes with their clinician.

Important safety information

Before starting therapy, review kidney function, electrolyte status, and blood pressure. Monitor potassium, sodium, magnesium, and uric acid during treatment; report signs of dehydration or dizziness promptly. Caution is advised in pregnancy and during lactation.

Nonsteroidal anti-inflammatory drugs (NSAIDs) may reduce diuretic efficacy and BP control. Interactions with lithium and certain antihypertensives require careful management. Regular follow-up and laboratory testing guide safe use and dose adjustments. Advise patients on recognizing dehydration and electrolyte symptoms.

Common side effects

Frequent adverse effects include electrolyte disturbances such as hypokalemia and hyponatremia, along with dehydration and orthostatic symptoms. Patients may notice increased uric acid with a risk of gout, and photosensitivity with sun exposure. Mild changes in glucose tolerance can occur, especially in susceptible individuals.

Less common but important reactions include fatigue, dizziness, and cramps. Seek urgent care for severe dehydration, persistent vomiting, signs of electrolyte imbalance, uric acid flare, severe allergic reaction, or chest pain. Patients should promptly report any unusual or persistent symptoms.
