Modafinil was first synthesized in 1976 by the French company Lafon as a wake-promoting agent. Since then it has achieved widespread use for narcolepsy, shift-work sleep disorder, and obstructive sleep apnea with daytime sleepiness.
Modafinil is a non-amphetamine wake-promoting agent used to improve alertness in excessive daytime sleepiness disorders. Its exact mechanism is not fully defined; it appears to raise central catecholamine signaling via dopamine transporter inhibition and to augment activity in orexin and histaminergic pathways. The drug is taken orally and prescribed by clinicians in standard tablet forms.
Pharmacokinetics show onset in 30–60 minutes, peak effect in 2–4 hours, and a mean half-life of about 12–15 hours. Metabolism occurs in the liver with active metabolites contributing to effect duration. Caution is advised in hepatic impairment and in individuals sensitive to stimulant exposure.
Modafinil is approved for narcolepsy with excessive daytime sleepiness, and for obstructive sleep apnea/hypopnea syndrome with daytime sleepiness. It is also approved for shift-work sleep disorder when daytime dysfunction is present. In some settings, clinicians may use it off-label for residual sleepiness related to other sleep disorders under supervision.
Timing: Take the dose in the morning for narcolepsy or sleep apnea. For shift work, take 0–1 hour before the start of the shift. Avoid late-day dosing to minimize insomnia. Dose: usual adult range is 100–200 mg once daily; dose may be adjusted up to 400 mg daily under medical supervision. Do not exceed the prescribed amount.
Food and administration: Modafinil may be taken with or without food. Some individuals tolerate it better with food; others prefer fasting. If a dose is missed, take it as soon as remembered unless it is near the time of the next scheduled dose; do not double-dose. Storage: store at room temperature (about 20–25 C) in a tightly closed container, away from moisture and out of reach of children.
Common adverse effects include headache, nausea, insomnia, dizziness, dry mouth, and anxiety; these are usually mild and transient. Serious reactions are rare but require prompt medical attention, including severe rash with swelling or trouble breathing, chest pain, signs of liver injury, or mood changes that are persistent or alarming. Pregnancy and breastfeeding require medical advice before use. Modafinil can interact with other medicines and may affect hormonal contraception; discuss all medications with your clinician. If symptoms persist or worsen, contact a healthcare professional.
